THIS IS THE REQUIRED PAPERWORK FOR COMPETITION TEAMS.
Attention Parents and Cadets due to liabilities/Medical issues that can arise at an event where parents are not present the following forms will be required for all participating Cadets. I know this is a lot of paperwork, but please understand that we participate in competitions in many counties that require different paperwork.
Although a couple forms are redundant, the reasons are;
The FSHAA forms are for Palm Beach County Schools.
The forms labeled Annex H are for Martin County, the State Raider meet, The National Raider meet.
Failure to provide the required paperwork will cause your child to not practice / compete on the particular teams. NO EXCEPTIONS!!
